Better reporting
Twitch should implement better reporting tools, starting with no longer requiring you to either report from within your channel or from their channel. For example, if reporting due to behaviors off-platform or reporting off-stream after gathering information to make the report rather than interrupt the flow of your own content, it's a bad experience to have to go their channel. If the person being reported is a streamer themselves, then that also forces someone to have to see that person's stream content or potentially live content in order to make a report, and because many people tend to wait to report, that may be a major barrier to reporting and add to what might be a traumatic experience already. There should be a portal for reporting. Additionally, because Trust & Safety apparently doesn't accept screenshots over concerns about fakes, the portal should somehow capture everything on Twitch and maybe even have a way to authorize Discord and Twitter to capture messages sent there as well. Otherwise, Twitch is basically wasting our time collecting and submitting evidence of TOS violations that seem to never get acted on, and that doesn't work. Reporting should also include additional categories, such as lewd or inappropriate behavior, because people may choose not to report because they don't identify their experience as harassment, while they may see it as inappropriate or breaking Twitch Community Guidelines.
