Removal of inactive users from follower Lists etc.
As I can see for myself and on the numbers under the streams I watch I noticed that often the number of follows is about 100x higher than the actual views and so I would recommend to remove inactive users in a process similar like follows:
After not watching a followed streamer for let's say 1 month the user gets a notification email that he is inactive for this specific channel and can be unfollowed if inactive for much longer.
After let's say 2 or 3 months of not watching a channel the user gets an email that states that if he/she does not watch this channel once in the next two weeks, he will automatically be unfollowed from this streamer.
After the threated amount of time passes without reaction the user will be unfollowed from the regaring streamers channel.
A settings option can be added that allows restoring single follows that have been undone manually or automatically. (For security reasons and if one does not have access to the internet for a long time)
This system should be balanced regarding the time limits etc. but I think that it could work just fine for giving streamers a better overview of the general amount of active people at all.
But the feature should be aware not to toggle if a streamer does not stream for a longer time so he/she does not automatically lose viewers because of being inactive.

Honestly I would rather have a tool, as a viewer, to unfollow streamers who haven't streamed in X months.

I don't want this to happen at ALL. I follow a LOT of people, and I do so for a reason because even though I may not watch all of them, I still want to be able to show them my support. And let's say after a year I see this person I have been following is live and I'm like "Hey, I haven't seen this person in awhiel, I should check up on them see how they're doing" If Twitch automatically unfollows that channel, that can't happen and it can cause issues. So no, I don't want this to happen. I want the people I follow to stay followed. I will stick with manually unfollowing them if I see it necessary, but I don't want a robot controlling the people I follow.

Honestly, this is a dumb idea. Following isn't only used to watch streamers. It is to support streamers as well. I've often followed stream I don't watch regularly watch just because I found them entertaining when one of the streams I visited often wasn't on. Just because I don't want to watch them right now doesn't mean I won't want to watch them again later. Followers also help introduce the streamer to other viewers. I sometimes browse the followed list of some of my twitch friends to find new streams to watch. The only thing your suggestion would do is greatly reduce many people's follower numbers and possibly stunt the growth of some channels.
