and restricting who can make an account would be a terrible idea from Twitch's perspective. Remember Adam Orth?

Just seems too insignificant for Twitch to bother with. I mean it might be neat to have some kind of indicator in chat to remind you you're a mod, but that's it.

Could you elaborate? How would this be beneficial?

For the time being, Chatty indirectly supports this. You can add a badge to people in a friends list.

...what's a username refresh?

Accounts that have had heavy activity would probably be best to never allow to be opened. Accounts with moderate activity could be about a year (this happen and you can't go on). Light activity about 6 months, and virtually no activity about 3 months.
Possibly allow account to 'bench' their accounts if they're going away for a while, which would triple the duration of how long until it's freed.

We apply IP bans in Chat based on users' Moderation actions in certain cases. We don't currently plan to publicize the logic we use because we think it would make it easier for bad actors to bypass the restriction.

If someone with different accounts keep coming into chat, that's ban evading. Report each account as they come in and the admins will catch on quick, probably global banning them and their IP.
I'm not on staff so I'm not 100% on this, if Xangold could confirm/deny.
I thought of a way to implement something like this without it being draconian. Similar to the option "Require a verified email to speak", there could be one for phone number.
