Hi, thanks for flagging this and sharing your suggestion.
We share and understand your concerns about how the intentional, repeated misuse of tags can undermine the purpose of the tags product itself. Tags are an important discoverability feature for communities, and a powerful way for all Creators—but especially those that are underrepresented—to express their personal identity and creative vision for their content.
We also do not feel Twitch should be the judge of Creators’ personal or creative identity, so we typically only enforce against the misuse of tags when it is associated with other behavior that violates our Community Guidelines such as hateful conduct or harassment.
That said, as the VTuber community’s experience demonstrates, there are limitations to this approach and we are actively investigating additional steps we can take to protect and improve the integrity of the tags feature at scale, without infringing on good faith Creators’ personal choice…

10/1/21 is just around the corner so it’s a certainty that Twitch is about to be filled with blood, guts, and gore.
The mature channel warning filters out children but there are many adults who find these images disturbing and wish to avoid them. Well, we have the tags, why not use them?
I personally don’t want to go into any channel unprepared for what I might see as I’m a bit of a snowflake. I had no idea for example that (SPOILER - click more to read the rest.)
Heavy Rain, a detective game, had you control someone as they cut off their own finger with a hand saw until I watched it in a live stream. I also saw a man get beaten to death with a camcorder in a stream categorized as “comfy.”
Point being, it would be really helpful to people like me if horror games and similar content were properly labeled. I don’t have PTSD or Epilepsy or anything but I know plenty of people can get very damaged by seeing this content when they are unprepared.
I know, I know, it’s an inconvenience to have to make sure graphic content is labeled as such but it’s a minor one, right? It would really help some people if it was done.
