The single easiest thing to vastly improve accessibility on Twitch, is remove compulsory autoplay. Add an easy-to-find setting that disables the autoplay feature.
Or a pop-up that asks if you want t continue seeing autoplaying content when you pause one of the many mini video players strewn haphazardly across the site. These clutter the website, make navigation harder, and make it dangerous for disabled people of many varieties to use Twitch.tv
If someone is epileptic and carefully selects channels which are epilepsy-safe, they can never be sure that Twitch won't just autoplay strobelights onto their screen.
If someone experiences joint or muscle pain, like in the case of early onset arthritis, then constantly having to scroll up and down pages to mute and pause videos can be extremely painful and taxing.
If someone is autistic or hypersensitive, and Twitch.tv decides it's a good idea to autoplay a stranger screaming at maximum volume, that can be an immediate day-ender. A meltdown due to unnecessary overstimulation will see extreme stress and pain placed on what is a not insignificant part of the userbase.
The decisions about which features to include in your website are not just about packing as much HTML into the page as possible. Leave room for users to breathe, or they will seek content elsewhere.

Scroll bars that disappear or lose functionality when you are not already scrolling defeat the point. The scroll bar is so small that it is literally impossible for me to click on. It does not even reappear when I mouse over where it should be.
The scroll bar is an accessibility feature, and making it "sleaker", or more "discreet" literally lowers the accessibility of Twitch.
